Understanding Your Insurance
All insurance companies work differently. It is essential that you understand what insurance coverage you have and what you need to do to make the most of your coverage.
Here are some helpful hints for you to ensure that you receive the benefits that you are entitled. We have also included a section for you to record the information you receive from talking with your insurance representative.

Alpenglow Acupuncture LLC is a Preferred Provider for Premera Blue Cross, Federal Blue Cross and ODS.
Alpenglow Acupuncture Payment Policies
Payment - We accept cash, personal check, Visa, Mastercard and Discover. Payment is expected at the time of service. Insurance is billed as a courtesy. Co-payments and deductible amounts are due at time of service.
No Show and Late Cancellation Charges - If you fail to cancel an appointment and do not notify the clinic you will be charged for the treatment. We request 24-hour notice of need to reschedule, less than 24 hours we will need to charge you $100. With enough notification we can usually fill the space you have made available for someone else.
Are Herbs and Supplements covered by Insurance? Herbs and Supplements are typically not covered by commercial insurance plans. Please check your plan provisions or speak to your insurance representative if your specific plan does cover Herbs & Supplements.
Insurance Definitions
- Co-pay
- The amount a person with insurance is expected by their insurance plan to pay at the time of service. This can be a percentage of the total charge or a set amount.
- Deductible
- The amount of money a person with insurance is required to pay out of pocket before insurance will begin paying toward charges.
- Prior Authorization
- Pre approval of visits by the insurance company.
- Yearly Benefit Maximum
- Maximum amount the insurance company will pay per year per insured.
Know your Benefits
Please call your insurance company before your initial visit to ensure eligibility of coverage. Some insurance companies require pre-authorization; it is your responsibility to obtain this authorization before your appointment. We have provided space below to record the information you receive.

- Be aware that Acupuncture coverage is often times different from your medical coverage.
- Always check to see if your provider is a preferred provider with your insurance company.
- Make sure you are in contact with your insurance to ensure your claims are paid correctly.
- Our billing office submits your insurance as a courtesy; it is your responsibility to follow-up with your insurance company to ensure payment.
